Medication Form - use if scout will be receiving medication while away with the troop.

Class 1 and Class 2 Medical Forms
- Class 1 is required each year for scouts and adults. (Informational only, no physical.)
- Class 2 is required for scouts for activities longer than 72 hours (including summer camp), but not high adventure (see Class 3 below) (Requires physical by doctor.) (Good for three years)

Class 3 Medical Form
- Class 3 is required for anyone over 40 years of age to partcipate in activities longer than 72 hours (including summer camp) and for scouts participating in High Adventure activities (Requires physical by doctor.) (Good for one year)
